Visit Reason
The inspection was a fire safety inspection conducted to assess compliance with fire safety regulations including exits, exit signs, fire alarm systems, hazardous area requirements, flame resistance of curtains and drapes, and electrical wiring maintenance.

Findings
The facility failed to maintain unobstructed exits, proper exit signage, operational emergency lighting, complete fire alarm system testing, self-closing smoke partition doors, flame-resistant curtains, and safe electrical wiring. These deficiencies affected all 43 residents present during the inspection.

19 CSR 30-86.022(7)(A) Exits-2 per Floor-Remote/Unobstructed. The facility failed to maintain unobstructed exits; the exit door on C Hall was sticking and the exit door on E Hall did not close properly.
19 CSR 30-86.022(8)(B) Exit-Directional Indicators. The facility had an exit sign with illuminated directional arrows indicating turns that were not present.
19 CSR 30-86.022(8)(C) Exit Sign-Illumination. The facility failed to ensure all emergency lighting/exit signs were operational; an exit sign/emergency light on B Hall failed to illuminate when tested.
19 CSR 30-86.022(9)(C) Fire Alarm System-Test/Maintain. The facility failed to ensure the complete fire alarm system was tested and maintained; the fire alarm for the Arbors was in trouble mode.
19 CSR 30-86.022(10)(A) Hazardous Area Requirements. The facility failed to install and maintain self-closing smoke partition doors; the laundry room door was not self-closing.
19 CSR 30-86.022(13)(D) Curtains/Drapes, Flame Resistant. The facility failed to ensure curtains were certified or treated to be flame-resistant; a curtain in the administrative assistant's office was untreated.
19 CSR 30-86.032(13) Electrical Wiring, Maintained, Inspected. The facility failed to ensure flexible power cords were not placed under rugs or in locations subject to damage; a flexible power cord was taped to a rug across a walkway in room C-9.
