Visit Reason
The inspection was conducted as an annual survey of Garden Villas South Assisted Living to assess compliance with state regulations regarding personnel records, resident medication reviews, orientation training, and emergency response procedures.

Findings
The facility was found deficient in maintaining required written physician statements for employees, documenting orientation training, completing monthly medication reviews for residents, and providing required dementia and emergency response training to staff. Several personnel files and resident records lacked proper documentation as required by regulations.

19 CSR 30-86.047(20)(I) Personnel Record-physician statement. The facility failed to ensure employees had a written statement by a licensed physician or designee indicating they could work in a long-term care facility for five of five sampled employees.
19 CSR 30-86.047(20)(K) Personnel Record-orientation training. The facility failed to ensure personnel files included documentation of orientation training for three of five sampled employees.
19 CSR 30-86.047(58)(B) Resident Condition/Medication Review. The facility failed to complete required monthly medication recapitulations for three of five sampled residents.
19 CSR 30-86.047(62)(B) Orientation-emergency response procedures. The facility failed to ensure staff received orientation on emergency response, infection control, resident dignity, abuse/neglect, and other required topics for two of five sampled employees.
