A subsequent case‑management visit on June 16 2021 was prompted by these findings. The licensing team confirmed the unapproved room conversions and the blocked exit, emphasizing the need for corrective action. Plans of correction were issued with due dates in late June 2021, requiring written notification of the changes, procurement of proper permits, and removal of obstructed egress routes.

While the report did not list new deficiencies, the pattern of earlier violations underscores the importance of maintaining accurate documentation, securing proper permits for structural changes, and preserving unobstructed exits.
