Inspection History
Inspection Report Summary for Green Valley Home Care
Green Valley Home Care, a six‑resident dementia and hospice‑support group home in Simi Valley, was the focus of two separate California Department of Social Services (CCLD) visits in late 2024. The first visit, conducted on 4 October, was a compliance‑verification call (COMP II) between the facility’s administrator, Vanessa Zanders, and a Centralized Application Bureau analyst. The call confirmed that the administrator had identified the facility, reviewed the required photo ID, and demonstrated a clear understanding of Title 22 regulations, including staffing qualifications, abuse prevention, medication management, incident reporting, and financial and criminal‑background requirements. The administrator was instructed to submit a signed LIC 809 form with copies of photo ID to complete the electronic submission process. No deficiencies were noted.
The second visit, a pre‑licensing inspection on 10 December, was led by Licensing Program Analyst Brian Balisi. It coincided with a change of ownership and the addition of a dementia program to the plan of operation, as well as a pending hospice waiver request. The inspector toured the single‑story facility, noting five resident bedrooms (one shared), two bathrooms with non‑skid showers, grab bars, and adequate temperature control, as well as fully stocked first‑aid kits, fire‑safety equipment (hard‑wired smoke and carbon‑monoxide detectors, serviced extinguishers), and a central medication storage cabinet. While the property lacked fire sprinklers and dedicated fire doors, the fire clearance had been granted for six non‑ambulatory residents, and all required exit routes and emergency signage were present. Kitchen and common areas were clean, properly equipped, and free of toxins or vermin. The inspector reported no corrective actions required, and the pre‑licensing Component III was completed with a formal exit interview. The facility was advised that operations could not commence until the CAB approved the license.
Overall, the reports reveal a facility that satisfies regulatory standards across physical, medical, and administrative domains. The lack of fire sprinklers and fire doors, though addressed through a clearance, remains a noteworthy safety gap. The repeated emphasis on staff training, documentation, and safety equipment underscores a pattern of proactive compliance. The next step for Green Valley Home Care is to await CAB approval, after which it may begin licensed operations.
