Visit Reason
The inspection was conducted to evaluate compliance with regulatory requirements related to notification of Medicaid/Medicare coverage and financial liability, and medication storage and security in the nursing facility.

Findings
The facility failed to ensure an Advanced Beneficiary Notice (ABN) was provided within two business days prior to discharge for one resident, and failed to secure medications properly as medication carts were left unattended with medications on top for two medication carts. Both deficiencies were determined to cause minimal harm or potential for actual harm.

Visit Reason
The inspection was conducted to assess compliance with medication storage and labeling, staffing payroll data submission to CMS, and infection prevention and control practices at Greenhurst Nursing Center.

Findings
The facility was found deficient in proper medication storage and labeling, failing to ensure expired medications were removed and labeled correctly. The facility also failed to submit mandatory staffing payroll data to CMS in the required uniform format. Additionally, staff failed to perform proper hand hygiene when handling clean linens, risking contamination and infection spread.

Visit Reason
The inspection was conducted as an annual survey to assess compliance with regulatory requirements for Greenhurst Nursing Center, including review of resident assessments, care practices, medication administration, and facility policies.

Findings
The facility was found deficient in several areas including inaccurate Minimum Data Set (MDS) assessments, failure to refer a resident for Level II PASARR evaluation, improper feeding tube placement checks, inadequate respiratory care and nebulizer equipment storage, and improper medication storage and labeling. These deficiencies were associated with minimal harm or potential for actual harm to residents.
