Visit Reason
The inspection was conducted as a survey of the Assisted Care Living Facility resulting in deficiencies cited for failure to provide safety, appropriate meals, maintain food supply, maintain a clean kitchen, and maintain medical records. A revisit survey was also conducted to verify corrections.
Findings
The facility was found deficient in multiple areas including resident safety, food service, kitchen sanitation, and medical record maintenance. Several deficiencies were re-cited during the revisit survey, and monetary penalties were assessed.
