Complaint investigations in recent years substantiated issues with supervision and elopement risk management, as well as timely reporting and investigation of abuse allegations. Enforcement actions included denial of payment for new admissions in 2016 and 2017 due to immediate jeopardy findings related to abuse and safety concerns, but no fines or license suspensions were listed in the available reports.
