Inspection Report Summary for Heritage Residential Care

The California Department of Social Services conducted an unannounced pre‑licensing inspection of Heritage Place, a 12‑bed residential care facility in San Mateo, on August 12, 2025. The visit, led by Licensing Program Analyst Jaime Vado, included meetings with administrator Villarose Gratuito and the new licensee, Katie Eiseman. The agency verified that the facility is a single‑level structure with no hospice residents at the time of inspection, and that its operating capacity of ten current residents is consistent with licensing requirements.

During the tour, the inspector noted that the kitchen, storage areas, and laundry facilities were clean and well maintained, with knives and medications securely locked. Safety equipment—including fire extinguishers, smoke and carbon monoxide detectors, and a full sprinkler system—was present and inspected as of 2024. Resident rooms were tidy, furnished appropriately, and shared half‑bath facilities were functional. Documentation reviews showed that resident files for eight of the ten residents and staff files for four employees were up to date, and the administrator’s certification was valid through April 2026. The facility’s emergency exit routes were unobstructed, and the last drill was conducted in June 2025.

No deficiencies or citations were identified during the inspection. The inspector’s report was reviewed with the administrator and licensee on the same day, and a copy was issued for public review. The Licensing Program Analyst notified the Community Care Licensing Division of the completion of the pre‑licensing inspection, and no corrective action is required. The facility remains in compliance with applicable statutes and regulations, presenting no immediate or potential risks to the health, safety, or rights of its residents.
