The November 2022 annual review repeated the infection‑control focus, noting adequate hand hygiene supplies, cleaning, and a designated infection‑control lead. All residents and staff were fully vaccinated, and no COVID‑19 cases were reported. The LPA did not raise any new deficiencies, but reaffirmed the facility’s compliance with current COVID‑19 protocols. The review also confirmed that weekly testing of unvaccinated staff was ongoing and that symptom screening and temperature checks were consistently performed for residents, staff, and visitors.

In March 2023, a case‑management visit addressed a complaint about a resident (R1) who sustained a head injury after being pushed by a staff member (S1). Video surveillance, resident and staff interviews, and an incident report were reviewed. The LPA noted that the recording captured a confrontation between the resident and an unidentified individual, with the staff member falling but not appearing to push the resident. The investigation concluded that no violation could be confirmed at that time; however, the facility was advised to submit additional evidence if available. This incident underscores a pattern of unresolved safety concerns beyond infection control, highlighting the need for ongoing monitoring of staff conduct and resident protection.
