The claim that records were withheld was addressed after the evaluator discovered that the requesting family member was not the resident’s authorized representative; the authorized representative denied making any request. In the May investigation, staff explained that family members who called to speak to a newly admitted resident could only reach a resident during routine call‑back periods because the resident was sleeping or engaged in an activity. No policy or directive prohibited such communication.
The final investigation, conducted in June, was **Substantiated**. The evaluator found that staff failed to secure two physician orders for blood‑pressure medication and an antibiotic that the resident needed. Review of medication administration records, physician orders, and staff interviews revealed that the resident ran out of Amlodipine 2.5 mg in early May and that an antibiotic for a urinary tract infection was delayed until after the resident’s symptoms had appeared. This mismanagement violated state medication‑administration standards and raised serious concerns about the facility’s oversight of resident care.
Across the four inspections, Ivy Park at San Marino exhibited a pattern of repeated complaints that were largely dismissed, except for the clear medication‑management lapse.
