The July 11, 2024 pre‑licensing evaluation of IVY PARK AT WOOD RANCH, administered by LPA Martha Arroyo, confirmed that the facility met all fire safety, food service, and accommodation requirements. The building received an approved fire clearance for its capacity of 100 residents, and the kitchen, dining areas, and laundry facilities were noted as clean, well‑stocked, and free of pests or toxins. All safety devices—including smoke alarms, carbon monoxide detectors, and sprinklers—were functioning, and the common areas were properly furnished and lit. The report found no deficiencies, indicating that the facility was ready for operation under the new ownership led by Jeanne Skondin.
On May 12, 2025, LPA Brian Balisi conducted an unannounced investigation following a complaint that staff had failed to keep a resident’s room free of bed bugs. The investigation revealed that a bed bug infestation had been identified in one resident’s room (R1) on April 9 and was treated by Ecolab on April 11 and again on April 14. No new activity was observed during the inspector’s review of the room, adjacent spaces, or common areas, and the facility had instituted daily inspections of all resident rooms. Because the evidence did not demonstrate a pervasive problem, the allegation was deemed unsubstantiated.
A second complaint, investigated on June 27, 2025, raised the same concern. While a single bed bug was found in room 227 during an Ecolab service call on June 9, the infestation was promptly addressed, and no further bugs were reported thereafter. The inspector noted that the facility’s monthly Ecolab inspections had been supplemented with additional visits, and that routine checks had yielded no further signs of infestation. Again, the lack of corroborating evidence led to an unsubstantiated outcome.
