A June 18, 2021, license renewal found no deficiencies. An October 4, 2022, license renewal found no deficiencies. A March 19, 2023, initial licensing found no deficiencies. A May 30, 2023, relicensure survey cited one deficiency: registered nurse failed to monitor and supervise insulin administration and finger stick blood glucose monitoring for one unstable diabetic resident.

An August 10, 2023, license renewal found no deficiencies. A June 25, 2024, relicensure survey found no deficiencies. A July 15, 2025, licensure survey with complaint investigation substantiated allegations of abuse, inadequate care, failure to ensure residents’ choice of activities, and failure to provide meals. Six deficiencies were cited: food items not covered or labeled with open and use dates, failure to provide sufficient staff for 14 residents not capable of self-preservation, failure to provide registered nurse supervision for eight residents for skilled nursing interventions, failure to ensure correct preparation for non-crushable medications, failure to ensure two staff received abuse training within 90 days of hire, and failure to conduct registry screening and background checks for two staff upon hire.

A November 3, 2025, complaint investigation addressing residents’ right to privacy, emergency evacuation staffing, freedom from abuse, and elopement prevention found no deficiencies. A November 16, 2025, complaint investigation substantiated failure to prevent resident elopement resulting in immediate jeopardy. One resident eloped using a secured employee code for the rear delivery door and was found on a busy street despite prior knowledge of the resident’s exit-seeking behavior. Immediate jeopardy was lifted after verification of corrective actions including enhanced supervision, door security upgrades, staff training, and medication adjustments with completion due December 17, 2025.
