Findings
The facility had multiple deficiencies across several categories including administration, nursing, resident rights, kitchen/food services, and environment over the last three standard surveys. Deficiencies ranged in severity with some rated as D and E, indicating issues in care planning, infection control, resident assessments, and medication error rates.

F 880 — The facility must establish and maintain an infection control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of disease and infection.
