Visit Reason
The inspection was an unannounced required annual visit conducted by the Licensing Program Analyst to review resident and staff records, inspect the physical plant, and ensure compliance with licensing requirements.
Findings
The inspection found several technical violations including missing physician reports/medical assessments for two residents, outdated appraisals/service plans for one resident, missing preadmission appraisals for three residents, and missing staff records including training. Additionally, the hot water temperature in restrooms was measured at 133.0°F, posing an immediate health risk, which was corrected during the visit. The facility was otherwise clean, safe, and properly furnished.

Visit Reason
A pre-licensing visit was conducted for a Change of Location (CHOL) application to license the facility as a Residential Care Facility for the Elderly (RCFE) with a total capacity of six non-ambulatory residents.
Findings
The facility was inspected for fire safety, personal accommodations, services, and food service. All safety equipment including smoke alarms, carbon monoxide detectors, and fire extinguishers were functional. The physical plant and environment were found to be in compliance with Title 22 regulations with no residents currently residing in the facility.
