The inspection was conducted as a renewal review of the facility's license by the Pennsylvania Department of Human Services, Bureau of Human Service Licensing on 09/18/2024.
Findings
The inspection found several deficiencies including outdated furnace inspections, staff training gaps related to transportation, expired first aid kit supplies, medication labeling errors, medication record inaccuracies, and incomplete resident records lacking recent photographs. Resident 1 is prescribed medication, but the pharmacy label on the medication container was incorrect.
According to resident 1 who self-administers, a medication was discontinued last year but is still listed on the medication administration record.
