Visit Reason
The inspection was conducted based on complaints regarding failure to investigate and timely report an injury of unknown source to a resident, and failure to ensure appropriate use and documentation of psychotropic medication.

Complaint Details
The complaint investigation focused on allegations that the facility failed to investigate and timely report a suspicious injury of unknown source on RI #64 and failed to properly manage and document the use of PRN antipsychotic medication for RI #43. The injury on RI #64 was observed on 7/17/2019 but was not reported to the State Agency until 1/21/2020. The facility acknowledged no investigation was conducted. RI #43 was administered Haldol as needed for several months without documented rationale or re-evaluation.
Findings
The facility failed to investigate a suspicious injury of unknown source on Resident Identifier (RI) #64 and failed to timely report the injury to the State Agency. Additionally, the facility failed to ensure that Resident Identifier (RI) #43 was not administered a PRN antipsychotic medication for more than 14 days without documented rationale or re-evaluation.

The facility was found deficient in ensuring a resident's suprapubic catheter was properly covered for privacy, following prescribed dietary restrictions for a resident, and properly labeling and sealing frozen food items in the kitchen freezer. These deficiencies posed minimal harm or potential for actual harm to residents.
