Visit Reason
This document is a Consent Order resulting from enforcement action by the Tennessee Board for Licensing Health Care Facilities following an annual follow-up life safety survey conducted on January 6, 2020.

Findings
The facility was found to have multiple life safety deficiencies including the presence of a portable space heater in a resident room, failure to provide documentation for the 2019 annual fire extinguisher inspection, and failure to provide documentation for quarterly fire drills for all shifts in 2019.
