Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Loyalty Home Health Care LLC, a provisional Assisted Living Facility licensee.
Findings
The licensee was found in substantial compliance but had several deficiencies including prohibited waiver of liability language in contracts, failure to provide required emergency relocation notices, incomplete training for unlicensed personnel, medication management issues, and restrictions on resident rights through house rules.
