Visit Reason
The Minnesota Department of Health conducted a full survey on June 3, 2025, to evaluate and assess compliance with state licensing statutes for Meadow Ridge Senior Living, an assisted living facility with dementia care license.
Findings
The survey identified multiple deficiencies including food safety violations, infection control issues, tuberculosis screening noncompliance, fire safety code violations, incomplete fire safety and evacuation plans and training, incomplete staff orientation and annual training, and late resident reassessments.

Food was not prepared and served according to the Minnesota Food Code, including cold holding temperatures above 41F and lack of sanitizer concentration in kitchen sinks.
Failed to establish and maintain an infection control program compliant with accepted standards, specifically related to hand hygiene by an employee.
Failed to ensure an employee's TB chest X-ray was completed within 90 days of a documented positive TB test.
