Staff and residents consistently denied incidents of disrespect or neglect, and the Administrator confirmed that safety measures, such as ADA‑compliant grab bars, met regulatory standards. The lack of evidence again resulted in an unsubstantiated outcome.
Across all three investigations, a pattern emerged: residents and families voiced concerns related to financial abuse, governance, and resident‑staff interactions, yet the facility’s records and staff interviews failed to substantiate these claims. The repeated unannounced visits underscore the importance of maintaining robust documentation and clear communication with residents, while also highlighting the challenges of verifying subjective allegations in a group‑home setting.
