In the 12/27/2022 investigation, Evaluator Kruz Long examined documentation and interviewed staff and residents regarding a claim that one resident was financially abusing another. In the 8/11/2023 investigation, Evaluator Erik Zaragoza followed up on complaints about maintenance delays, staff disrespect, and unmet resident needs. The facility’s work‑order system showed that maintenance requests were addressed within a week, contrary to the claim of a six‑month delay. Staff and residents consistently denied incidents of disrespect or neglect, and the Administrator confirmed that safety measures, such as ADA‑compliant grab bars, met regulatory standards. The lack of evidence again resulted in an unsubstantiated outcome.

Across all three investigations, a pattern emerged: residents and families voiced concerns related to financial abuse, governance, and resident‑staff interactions, yet the facility’s records and staff interviews failed to substantiate these claims. The repeated unannounced visits underscore the importance of maintaining robust documentation and clear communication with residents, while also highlighting the challenges of verifying subjective allegations in a group‑home setting.
