The first assessment of Morningstar Senior Living of Mission Viejo was a COMP II interview conducted on May 1, 2023. Administrator Dyan Summerell confirmed the facility’s understanding of all applicable California health and safety codes, and signed the LIC 809 form. No deficiencies were recorded, and the facility was noted as a 740‑type RCFE with a capacity of 198 residents, pending final licensing.
A pre‑licensing visit on May 22, 2023 by LPA Ruth Martinez found the three‑story building fully prepared for licensure. The audit documented comprehensive safety systems—including delayed egress for memory care, smoke detectors, and an approved fire clearance—along with adequate medical, kitchen, and recreational facilities. All rooms were apartment‑style and met resident and staff space requirements. The evaluator concluded the facility was ready for licensure, and the file was forwarded for approval.
Beginning in July 2024, the agency began a series of collateral inspections tied to complaints filed at other licensed homes. An unannounced visit on July 15, 2024 by LPA Joseph Alejandre verified the presence of required signage (See Something, Say Something) and requested physician reports from the resident implicated in the external complaint. A January 30, 2025 visit by LPAs Kimberly Lyman and Fred Arias followed a similar procedure, interviewing the resident and obtaining relevant documentation. On June 6, 2025 LPA Kimberly Lyman again conducted a collateral interview with a resident and concluded the visit with an exit interview. In each case, no operational or safety violations were documented; the visits served to gather information for the external complaints.
Across all reports, Morningstar Senior Living of Mission Viejo consistently demonstrated compliance with California licensing requirements. The facility maintained accurate resident records, adequate safety and emergency plans, and the required training and staffing protocols. The repeated unannounced collateral visits illustrate ongoing regulatory oversight, yet the absence of noted deficiencies suggests stable, quality care and robust safety practices.
