Inspection Report Summary for MorningStar Senior Living of Pasadena

The California Department of Social Services Community Care Licensing Division conducted four unannounced investigations at Morningstar of Pasadena (Facility Number 198603416). In the first visit on December 10 2021, the evaluator determined the facility had repeatedly failed to submit the required weekly laboratory reports for unvaccinated staff and residents to the Pasadena Public Health Department between September and November 2021. The Executive Director acknowledged the oversight and instituted a dedicated team to ensure future compliance. Because the evidence met the preponderance of evidence standard, the allegation was **substantiated**, and a deficiency was cited under Section 87211(a)(2) of the California Code of Regulations, with a plan of correction due December 17 2021.

The second visit on April 21 2022 addressed an allegation that staff were not properly trained. Evaluator Cynthia D. Chan reviewed staff and resident rosters, training logs, and interviewed staff and residents. Documentation demonstrated that all care staff had completed the required training hours, and residents reported confidence in staff competency. Accordingly, the allegation was **unsubstantiated** and no corrective action was required.

The third investigation on January 11 2023 examined multiple complaints—falls, food poisoning, delayed assistance, and meal timing—arising from alleged supervisory and food‑service deficiencies. The evaluator gathered menus, staffing schedules, food‑handling training records, and interviewed eight staff members and eleven residents. Observations revealed that staff maintained appropriate ratios, performed rounds every two hours, and that kitchen staff complied with mask, glove, and hygiene protocols. Residents reported adequate supervision and timely meals. No evidence supported the allegations, leading to an **unsubstantiated** determination.

Across all four visits, the primary pattern is a single substantiated compliance issue regarding COVID‑19 documentation, while subsequent allegations of training deficiencies, falls, and food safety were found unsubstantiated. The findings underscore Morningstar of Pasadena’s recurring challenges with documentation and the need for ongoing oversight to prevent recurrence, yet also indicate that staff training and resident supervision generally meet regulatory expectations.
