Inspection Report Summary for Nikkei Senior Gardens
NIKKEI SENIOR GARDENS, located at 9221 Arleta Ave, underwent a series of inspections by the California Department of Social Services (CCLD) from August 2021 through January 2024. The first unannounced visit on 31 August 2021 was prompted by an incident report alleging a resident’s personal rights violation. During the tour the licensing program analyst observed no health or safety violations and noted that the facility’s public bathrooms, kitchens, resident apartments, and PPE storage met regulatory requirements. The visit ended with a follow‑up to be conducted, and the executive director was provided a copy of the report.
Subsequent unannounced “One‑Year Required Infection Control” inspections on 27 January 2022 and 19 January 2023 confirmed continued compliance with Title 22 regulations. Each visit documented adequate COVID‑19 prevention signage, screening stations, hand‑sanitizing stations, and personal protective equipment supplies. The facility maintained clear entryways, well‑furnished resident rooms, secure medication and toxin storage, and functioning fire safety equipment, including monthly smoke‑alarm tests and monthly fire‑drill drills. No deficiencies were cited in either inspection, and the licensing analysts noted that all beds, bathrooms, and common areas were clean, unobstructed, and properly furnished.
The most recent Annual Continuation Inspection on 2 January 2024 likewise reported no health or safety hazards. The executive director attended the visit, and the facility was reaffirmed as licensed for 66 non‑ambulatory and 40 bedridden residents, with a dedicated dementia wing and an approved hospice waiver for ten residents. Throughout the four reports, NIKKEI SENIOR GARDENS consistently demonstrated adherence to infection control protocols, fire safety standards, medication management, and resident‑care furnishings, with no significant complaints or violations noted beyond the initial 2021 incident that required further review.
