Noble Living II LLC, a six‑bed facility in El Cajon, underwent a series of unannounced inspections by the California Department of Social Services (CCLD) over the period from September 2021 to January 2023. The initial visits in 2021 and 2022, conducted by Licensing Program Analysts Alexandre Vo and Vicky Williamson, focused primarily on infection‑control compliance amid the COVID‑19 pandemic. Each evaluation confirmed that the facility’s epidemic outbreak plans, entry screening, universal symptom checks, hand‑hygiene stations, and personal protective equipment supplies met state standards; no deficiencies were cited in any of those visits.
In July 2022, two separate case‑management visits addressed routine administrative matters. The first noted a resident death that the facility had reported on June 29, while the second followed up on a self‑reported fall incident involving Resident 1 (R1). Both visits included staff interviews and record reviews, and the LPA found no operational deficiencies in either case. The fall incident involved R1 falling into a hallway, slipping in a shower, and later refusing to leave with emergency responders; paramedic assessment found no injuries, and the incident was reported to the responsible party.
The first substantive deficiency emerged in a case‑management visit on January 20, 2023, when Licensing Program Analyst Esther Miller reviewed post‑death administrative procedures. The facility had taken 19 to 21 days to issue refund checks after removing residents’ personal property—a period exceeding the 15‑day requirement in California Code of Regulations, Title 22, Division 6. The deficiency was cited under LIC 809D, and a plan of correction was jointly developed with the licensee. No other deficiencies were noted in the same visit, and the facility complied with all other regulatory obligations.
Overall, Noble Living II LLC demonstrated consistent compliance with infection‑control and incident‑reporting requirements, but the investigation highlighted a process gap in timely refunds after resident deaths. The Department’s corrective action plan focuses on aligning refund timelines with statutory deadlines to protect residents’ rights.
