Visit Reason
The inspection was conducted to assess compliance with medication administration regulations and certification requirements for medication aides at Northpark Village Assisted Living.

Findings
The facility failed to ensure a safe and effective medication system as a Level One Medication Aide administered medications with an expired certification. The certification and training documentation for the aide was incomplete and the Director of Nursing was unaware of the expired certification.
