Inspection Report Summary for Oakmont of Chino Hills
Oakmont of Chino Hills (Facility #361881162) was the subject of three unannounced Community Care Licensing inspections conducted between July and September 2023. Each evaluation concluded that the allegations raised were **unsubstantiated** and that no regulatory deficiencies were identified under Title 22 of the California Code of Regulations. Following each visit, an exit interview was held with Administrator Janeth Medrano and a copy of the report (LIC 9099) was provided.
The first inspection on July 6 addressed multiple complaints, including pressure injuries, inadequate incontinence care, inappropriate staff‑resident communication, rough handling, and staff providing care while under the influence of substances. Documentation review confirmed that four residents currently have pressure injuries, which are being managed by home health or hospice providers; the facility follows those providers’ instructions and does not treat the wounds directly. Staff and residents reported that diapers are checked regularly and that no rashes or sores developed, and no incidents of substance use were observed. The allegations therefore lacked sufficient evidence to be deemed substantiated.
The subsequent inspections on July 17 and July 16 focused on physical aggression and privacy violations. Allegations that staff pushed residents or handled them roughly were denied by both staff and residents, and no supporting evidence emerged. In the third report, concerns were raised that a staff member disclosed a resident’s confidential information and that insufficient supervision allowed a resident to sexually harass another via personal cell phones. Interviews revealed that staff acknowledged the incident but noted that residents are largely independent and manage their own devices; no policy breach or supervisory failure was found. Across all investigations, a consistent pattern emerged: staff and residents reported respectful interactions, diligent monitoring of incontinence and wound care, and no evidence of substance‑related impairment or abuse. The facility’s compliance record remained unblemished during the review period.
