Oakmont of Simi Valley (facility number 565850291) was the subject of two unannounced complaint investigations conducted by the California Department of Social Services Community Care Licensing Division. The first investigation, led by Evaluator Martha Arroyo and filed under control number 29‑AS‑20240411103658, was performed on 11/19/2024 following a complaint received on 4/11/2024. The second investigation, led by Evaluator Brian Balisi and filed under control number 29‑AS‑20250312141224, took place on 4/14/2025 after a complaint received on 3/12/2025. In both visits the Executive Director, Christina Spears, met with the evaluator, and the investigations involved staff interviews, resident file reviews, and physical plant inspections.
The 2024 complaints alleged that staff failed to seek timely medical care for a resident who sustained a skin tear and that staff handled a resident roughly, causing the injury. The evaluator reviewed incident reports, police findings, and staff statements. Staff reported that the resident’s skin tear was promptly cleaned and bandaged, that a 911 call was made the following morning, and that the family was notified and had paramedics dispatched. Interviews also confirmed that staff followed established procedures for injury reporting and family communication. No evidence was found to support the allegations, and both were deemed unsubstantiated. Similarly, a police investigation found no criminal conduct.
The 2025 complaints focused on inadequate food services, personal hygiene protocols, laundry services, and facility temperature. Interviews with eight staff members and three family representatives, coupled with observations of the kitchen and laundry areas, revealed that meals were timely and nutritionally appropriate, hand‑hygiene and protective equipment were consistently used, laundry was completed according to schedule, and temperature controls were maintained. No residents or families reported concerns, and all allegations were likewise unsubstantiated.
Across both investigations, the pattern is one of repeated complaints that, upon review, were not corroborated by evidence. The facility’s documented responses and operational practices indicate ongoing compliance with licensing standards.
