Oaks at Eagle’s Landing

The investigation was initiated due to complaint intakes #GA00248427 and GA00248461. The complaint was substantiated as the facility did not notify the family of Resident #1 about the bruised lip and swollen hand observed after the incident on 7/6/2024.

The facility failed to ensure immediate appropriate action was taken in response to an incident involving Resident #1 and Resident #2, including failure to notify the resident's family or legal representative about injuries sustained during the altercation.

Investigation was initiated based on intake #GA00247370. The complaint was substantiated by observation, record review, and staff interview confirming overcapacity.

The facility was found to be serving more residents than its approved licensed capacity, with a census of 88 residents exceeding the licensed capacity of 86 beds as per the permit issued on 4/4/2022.

Visit was complaint-related, investigating intake #GA00241252 and #GA00241234.

The facility failed to provide two direct care staff persons on-site in the memory care unit at all times, as required. Observations and interviews confirmed that at times only one staff was present while residents were unattended, and no replacement staff were assigned during lunch breaks.
