Complaint Details
The complaint investigation found substantiated violations related to medication administration and hospice care services resulting in endangerment to residents' health and safety.
Findings
The facility failed to administer blood thinner medication as ordered for Resident #1, failed to secure a physician certification for hospice services for Resident #2, failed to update Resident #2's care plan to include hospice services, and failed to reflect the evacuation method for Residents #2 and #4 during an emergency.
