Visit Reason
The inspection was a routine re-licensure visit to ensure compliance with licensing and regulatory requirements for an assisted living facility.

Findings
The inspection found several areas of noncompliance primarily related to staff training, medication administration, housekeeping, maintenance, and emergency preparedness. Some deficiencies were corrected during the inspection, while others required follow-up.

R432-270-14(a-g): Four oxygen tanks were unsecured in a resident room, posing a safety hazard.

R432-270-26(6)(a-k): The emergency and disaster plan did not assign specific tasks during emergencies and lacked instructions for fire and evacuation procedures.
