The inspection was conducted to assess compliance with regulatory requirements related to resident assessments, care planning, food safety, infection control, and overall facility operations at Palm Gardens Health and Rehabilitation, LLC.

Findings
The facility was found deficient in multiple areas including failure to accurately code hospice status on a resident's assessment, lack of care plans for recurrent UTIs and depression, improper handling and storage of food and utensils, failure of staff to wash hands properly in the kitchen, and inadequate infection prevention practices in the laundry department.

Deficiencies (6)
Failed to ensure Resident Identifier #30's Significant Change Minimum Data Set (MDS) Assessment was coded for hospice.
Failed to develop and implement care plans for recurrent UTIs and depression for Resident Identifier #22.
Spoons, forks, and knives were placed in utensil bags while wet, risking cross contamination.
Expired thickened waters (29 units) were found in the resident/supplement refrigerator.
Staff failed to wash their hands when entering and while in the kitchen, risking food contamination.
Laundry staff did not wear protective aprons while handling soiled linen and allowed clean linens to touch dirty uniforms while folding.
