Inspection Report Summary for Park View Place
On January 20, 2023, an unannounced complaint investigation was conducted at Oakmont of Covina Hills (facility number 198603545) following allegations that resident care needs were unmet because of insufficient staffing. The evaluator met with Administrator Patricia Gustin, interviewed all staff and a sample of residents, and reviewed documentation. Residents reported that their activities of daily living were adequately addressed, and staff confirmed they had sufficient coverage—at least three caregivers per unit on each shift. Documentation of care was monthly or bi‑weekly, with daily staff notifications. No evidence of a staffing shortfall was found, and the investigation concluded the allegation was unsubstantiated, citing no regulatory deficiencies.
On July 18, 2023, a separate unannounced visit was performed at Park View Place, also listed as facility number 198603545, to examine a complaint that Administrator Patricia Gustin was failing to meet administrative duties and qualifications. Interviews with residents, visitors, and staff indicated that Gustin spent the requisite hours at the facility and that staff training was current. However, a file review revealed that Gustin’s administrative certificate had expired on June 8, 2023, and renewal was not yet complete. Under California Code of Regulations, Title 22, the facility lacked a qualified and currently certified administrator, leading to a substantiated finding and a cited deficiency (CCR 87405(a)). A plan of correction required Gustin to provide proof of certification by August 28, 2023, to avoid penalties.
On August 28, 2023, the same evaluator issued an amendment to the July report, reaffirming the substantiated finding. The visit included a meeting with Business Office Director Karen Turnour and confirmed that an interim certified administrator, Sahar Mosalla, held a valid certificate until June 22, 2024. Despite this interim coverage, the primary administrative position remained unqualified, and the deficiency persisted. These reports demonstrate a pattern of regulatory scrutiny focused on administrative certification rather than staffing adequacy, with consistent findings of non‑compliance at the administrative level for Park View Place.
