Inspection Report Summary for Pepper Tree Assisted Living

Pepper Tree Assisted Living, a six‑bed, non‑ambulatory senior facility located at 2353 251st Street, Lomita, was inspected by the California Department of Social Services during a series of unannounced visits from November 2021 through November 2022. The first 2021 assessment, conducted by Licensing Program Analysts Ana Soto and Janae Hammond, found the facility’s infection‑control measures to be robust. PPE supplies were readily available, a sanitized station existed at the entrance, and all staff were wearing face coverings. The physical plant—single‑story with five resident rooms, four bathrooms, a dining area, and two shaded patio spaces—met temperature and cleanliness standards, and the resident and staff files appeared organized.

Later visits focused on administrative compliance. In April 2022, a plan‑of‑correction (POC) visit confirmed that the over‑the‑counter medications for Resident #1 had been labeled correctly and the earlier deficiency had been addressed. The July 2022 case‑management visit uncovered a criminal‑record‑clearance lapse: two staff members, Princess Diaz and Miguel Can, had not obtained the required California clearance, resulting in a $100 civil penalty for Diaz and a $500 penalty for Can. Both citations were cleared on the same day after the licensee, Stephanie Klein, submitted the necessary clearances. The November 2022 inspection repeated the infection‑control focus and found no new deficiencies, with all rooms and common areas compliant with Title 22 regulations.

Across the four assessments, the facility demonstrated a pattern of strong infection‑control practices and prompt remediation of administrative shortfalls. By the final inspection, all noted deficiencies had been corrected within the prescribed deadlines, and no additional civil penalties were imposed. The reports reflect a facility that maintains safe environmental conditions while progressively resolving documentation gaps in medication labeling and staff clearance.
