Visit Reason
The Commission conducted a complaint survey of Gallaway Health and Rehab nursing home on or about December 21, 2022, following allegations of resident abuse and related concerns.
Complaint Details
The complaint investigation was substantiated, finding multiple incidents of abuse and neglect involving six residents.
Findings
The facility failed to ensure six residents were free from abuse, including incidents of rough handling causing a fractured humerus, residents left unsupervised due to staffing shortages, physical altercations among residents, and failure to complete neurological assessments after an unwitnessed fall.
