No health deficiencies were found during the inspection.

The complaint investigation revealed that CNA1 attempted to transfer resident R9 alone using the wrong sling on 07/01/24, resulting in a fall and fractured clavicle. The incident was substantiated and corrective actions were taken including CNA1's termination.

The facility failed to ensure safe mechanical lift transfers for one resident out of six reviewed, resulting in actual harm. The incident involved improper use of the mechanical lift by a Certified Nursing Assistant (CNA1) who transferred the resident alone using the wrong sling, causing a fracture. The facility implemented corrective actions including staff retraining, discarding old slings, and terminating the responsible CNA.

The facility was found deficient in multiple areas including failure to remove expired medications and biologicals from medication storage rooms, improper food labeling and storage with expired and moldy foods found in refrigerators and dry storage, inadequate refuse disposal with uncovered grease bin attracting flies, and failure to ensure required in-service training for nurse aides.
