Visit Reason
The Department conducted a complaint survey investigation at Raintree Terrace from March 5, 2018 through March 7, 2018 following concerns about a resident with Alzheimer's dementia who eloped from the facility.
Complaint Details
The complaint investigation was substantiated based on findings related to Resident #9's elopement and safety failures.
Findings
The investigation found that Resident #9, who was at elopement risk, wandered and was missing for approximately an hour and a half before being found outside the building. The secured unit door did not alarm at the time of elopement, and staff did not hear the courtyard gate alarm.

Rule 1200-08-25-.07(7)(a)(2) requires each assisted-care living facility to provide safety for residents. The facility failed to ensure safety when Resident #9 eloped due to a non-alarming secured unit door and staff not hearing the courtyard gate alarm.
