Findings
The facility was found to have multiple deficiencies including failure to honor residents' rights regarding meal service, breaches in confidentiality, failure to follow physician orders, improper medication storage, inadequate respiratory care, failure to post nurse staffing schedules, improper food preparation and storage, and lapses in infection control practices.
