A plan of correction was issued, requiring the administrator to secure hospice‑provided medication training within three weeks and to conduct in‑house training by January 2023. The same visit addressed additional concerns about rough handling, hygiene, feeding, and administrator presence, but the evidence did not support those claims, and they were recorded as unsubstantiated. The report also noted deficiencies in facility repair and in the completion of required staff training, with a due date for corrective action.

The second investigation on 01/04/2024, led by evaluator Magda Malcore, examined separate complaints. A plan of correction was issued, requiring the licensee to submit proof of repairs by 01/26/2024. The evaluation also noted that the facility’s main heater had been nonoperational for two weeks and that a repairman had been scheduled to address the garage door. While the floor‑cleaning complaint was dismissed, the physical condition of the building emerged as a significant compliance issue.
