A plan of correction was issued, requiring the administrator to secure hospice‑provided medication training within three weeks and to conduct in‑house training by January 2023. The same visit addressed additional concerns about rough handling, hygiene, feeding, and administrator presence, but the evidence did not support those claims, and they were recorded as unsubstantiated. The report also noted deficiencies in facility repair and in the completion of required staff training, with a due date for corrective action.
The second investigation on 01/04/2024, led by evaluator Magda Malcore, examined separate complaints. The allegation that staff were not properly cleaning floors was unsubstantiated; floors appeared clean, staff and residents denied any issues, and residents reported overall cleanliness. A different allegation, that the facility was in disrepair, was substantiated. The evaluator observed a stand‑alone heater that was not functioning, missing and loose tile around the kitchen sink exposing rock and gravel, and a broken glass window in resident #1’s bedroom. These findings violated the requirement that the facility be clean, safe, sanitary, and in good repair. A plan of correction was issued, requiring the licensee to submit proof of repairs by 01/26/2024. The evaluation also noted that the facility’s main heater had been nonoperational for two weeks and that a repairman had been scheduled to address the garage door. While the floor‑cleaning complaint was dismissed, the physical condition of the building emerged as a significant compliance issue.
Across both investigations, a pattern of recurring deficiencies is evident. Medication management and staff training failures in 2022, coupled with unresolved maintenance problems in 2024, suggest that Rose Valley Redlands is struggling to meet regulatory standards for resident safety and facility upkeep.
