The purpose of this visit was to investigate complaint #GA00173542. An on-site visit started on 4/11/17 and the investigation was completed on 4/12/17.
Complaint Details
The investigation was complaint-driven based on complaint #GA00173542. The complaint was substantiated by findings that Resident #1 sustained a fractured finger due to inadequate supervision and lack of safety measures in the care plan.
Findings
The facility failed to provide supervision commensurate with the needs of one sampled participant, resulting in an injury to Resident #1's left index finger. The participant sustained a fracture, and the care plan did not address safety precautions related to the resident's habit of placing hands in wheelchair spokes.
