While R1’s medical records indicated ambulatory status and a walker was noted in the care plan, the incident reports failed to reflect the resident’s fall risk. The evaluator determined that the evidence met the preponderance standard, and the complaint was substantiated, pointing to deficiencies in staff training, protocol adherence, and documentation.
Across these investigations, a pattern emerges: the facility has experienced recurring safety concerns—first environmental, then staffing and documentation. While the 2023 findings were dismissed, the 2021 and 2024 reports confirmed substantive lapses, prompting formal deficiencies and requiring corrective action. Continued monitoring will be essential to ensure that both physical infrastructure and resident‑care protocols meet California’s stringent standards for safety and quality.
