The August 2023 investigation, conducted by Angelica Rea, addressed staff conduct claims that residents’ personal belongings were not safeguarded and that rooms were being broken into. Interviews with the administrator, staff, and residents, along with a review of resident files, produced no corroborating evidence. Both the staff and the resident interviewed denied the allegations, and no incidents of theft or unauthorized entry were documented. As a result, the complaint was ruled unsubstantiated; no corrective action or deficiency citation was issued.

In February 2024, evaluator Mary G. Flores investigated serious allegations of resident safety neglect, including multiple falls, failure to follow fall‑protocol, improper medication administration, and inadequate incident reporting. The investigation uncovered that resident R1 had fallen twice in January 2024, with incident reports that omitted any corrective measures or future fall‑prevention plans. While R1’s medical records indicated ambulatory status and a walker was noted in the care plan, the incident reports failed to reflect the resident’s fall risk. The evaluator determined that the evidence met the preponderance standard, and the complaint was substantiated, pointing to deficiencies in staff training, protocol adherence, and documentation.

Across these investigations, a pattern emerges: the facility has experienced recurring safety concerns—first environmental, then staffing and documentation. While the 2023 findings were dismissed, the 2021 and 2024 reports confirmed substantive lapses, prompting formal deficiencies and requiring corrective action. Continued monitoring will be essential to ensure that both physical infrastructure and resident‑care protocols meet California’s stringent standards for safety and quality.
