Visit Reason
The visit was an annual survey conducted by surveyors on or about December 16-17, 2019, to assess compliance with assisted-care living facility regulations.
Findings
Surveyors found violations related to food storage and labeling, including uncovered and undated food items, lack of sanitizer liquid for cleaning kitchen rags, and uncovered trash cans. The dietary manager failed to ensure proper food labeling and sanitation procedures.

The visit was a revisit survey conducted on or about April 1, 2019, to ensure that previously cited deficiencies from a January 29, 2019 survey were corrected.
Findings
The facility failed to meet the nutritional needs of residents with prescribed low concentrated sweets and no added salt diets. The cook and staff did not follow dietary orders, and residents were served inappropriate diets contrary to physician instructions.
