In the October 2021 and October 2022 visits, Licensing Program Analyst Cynthia Chan found the facility in full compliance with infection‑control protocols, resident and staff documentation, medication storage, and emergency preparedness. Staff wore face coverings, hand‑sanitizer stations were abundant, and all rooms inspected had appropriate furnishings and acceptable hot‑water temperatures (105–120 °F). The reports from these years recorded no deficiencies, indicating solid operational standards at those times.

The October 26 2023 inspection revealed a new issue: four of eight randomly selected rooms had hot‑water temperatures exceeding the mandated 120 °F, creating an immediate health‑safety risk. Cynthia Chan documented the deviation under CCR 87303(e)(2) and issued a deficiency, requiring the administrator to correct the temperature range and submit a log by October 27 2023. At that time, the resident census had grown to 52, underscoring that capacity increases had not yet been matched with updated environmental checks.

A separate November 7 2023 investigation, triggered by a resident‑care complaint, uncovered a documentation lapse. Licensed Program Analyst Alberto Lopez noted that Resident #1 lacked an up‑to‑date physician’s report (LIC 602) – a requirement that must be maintained within the past year.
