Visit Reason
Unannounced routine inspection to review compliance with nursing care facility regulations.
Findings
The inspection found four rule noncompliances related to various regulatory requirements including employee training, resident rights, medication administration, and restraint policies. Technical assistance was provided in several areas.
Deficiencies (4)
R432-150-4(3)(a-g) The licensee did not ensure a registered nurse serves as director of nursing on a full-time basis and nursing staff are present 24 hours a day to meet resident needs.
R432-150-13(5)(a-j) The licensee did not fully comply with policies governing physical and chemical restraints, including authorization, monitoring, and documentation requirements.
R432-150-22(13)(a-c) The licensee did not ensure dining assistants completed required background screening and training for residents without complicated feeding problems.
R432-150-31-4(2)(a-i) The licensee did not fully implement policies to ensure compliance with OLST requirements including admission determination, order changes, and staff training.
