The facility failed to properly maintain fire extinguishers, conduct required fire drills and consultations, maintain emergency lighting, use approved wastebaskets, and maintain electrical wiring. These deficiencies affected all seventeen residents present during the inspection.

19 CSR 30-86.022(3)(D) Fire Extinguishers UL/FM, Maintain/Check. The facility failed to properly maintain/install fire extinguishers in accordance with NFPA 10, 1998 edition. The class K fire extinguisher in the kitchen was improperly placed on the floor.
19 CSR 30-86.022(5)(A) Fire Drill/Evacuation Plan, Consultation. The facility failed to request the required annual consultation from the local fire authority and provide an up-to-date emergency plan to the local jurisdiction's emergency management director.
19 CSR 30-86.022(12)(C) Emergency Lighting - Battery Powered, 1.5 hrs. The facility failed to maintain emergency lighting in good repair; several emergency lights failed to activate during testing.
19 CSR 30-86.022(15)(A) Wastebaskets, Metal/UL/FM-Requirements. The facility failed to ensure only metal or UL/FM fire-resistant rated wastebaskets were used; unapproved plastic wastebaskets were observed in multiple resident rooms.
19 CSR 30-86.032(13) Electrical Wiring, Maintained, Inspected. The facility failed to properly maintain electrical wiring, presenting a safety hazard; an unapproved electrical extension cord was in use in a resident room.

The inspection was conducted to review compliance with fire drill requirements and emergency preparedness as part of a fire safety inspection.

Findings
The facility failed to perform and/or document the required number of fire drills on each shift every three months over the past twelve months. The deficiency affected all seventeen residents present during the inspection.
