Visit Reason
Follow-up survey to determine correction of orders found on the survey completed on May 29, 2024, which had not been corrected at the time of the August 12, 2024 survey.

Findings
The facility had multiple uncorrected deficiencies including training and competency evaluations for unlicensed personnel, orientation content missing for employees, insufficient dementia care training, lack of medication management training and competencies for unlicensed personnel, incomplete individualized treatment and therapy management plans, missing or outdated treatment orders, inappropriate care and services related to side rails, and fire safety and physical environment issues.

Failed to ensure training and competency was completed for unlicensed personnel (ULP-E) including personal hygiene, grooming, and other required content.
Failed to ensure orientation content was complete for unlicensed personnel (ULP-C, ULP-D, ULP-E) including principles of person-centered planning.
