A relicensure survey with complaint (#OK00084910) was conducted from November 18 through November 20, 2025, to assess compliance with state licensure requirements and investigate complaint allegations related to medication administration and storage.

Deficiencies were cited for failure to complete annual resident assessments for 2 of 8 sampled residents and failure to administer medications per physician orders for 1 of 8 sampled residents. The facility was found to have potential for more than minimal harm. A revisit was conducted on December 12, 2025, confirming that deficiencies were corrected as of December 9, 2025.

The facility failed to ensure residents were screened daily for possible symptoms of COVID-19 as recommended by CDC guidelines, which posed a potential for more than minimal harm. Temperatures were taken but symptom screening was not performed for three sampled residents.

The visit was complaint-related as a Covid-19 Special Focus survey to investigate infection control practices. The deficiency was substantiated based on interviews and record review.
