Inspection Report Summary for The Forum at Rancho San Antonio

The California Department of Social Services conducted a series of case‑management visits to Forum at Rancho San Antonio, a 741‑type assisted‑living community, during the first half of 2021. The initial two inspections in January and February were tele‑inspections prompted by the COVID‑19 shelter‑in‑place order. During those visits the licensing analyst reviewed a selection of villas, confirmed that all fire‑extinguishers were properly serviced, smoke‑and‑carbon‑monoxide detectors were operational, and that lighting, grab bars, and temperature controls met regulatory standards. The facility successfully applied for incremental capacity increases—first from 396 to 406 ambulatory residents, then from 406 to 408—each of which was approved after the virtual tours and documentation review. All villas inspected during these visits were deemed ready for occupancy.

In March, the analyst focused on a single two‑story villa (#64) while again performing a virtual assessment. The same safety criteria were verified: functional alarms, working elevators, properly maintained fire suppression equipment, and adequate kitchen and laundry facilities. The hot‑water temperature was within acceptable limits, and the elevator was noted to be in working condition. The report reaffirmed the facility’s readiness for the modest capacity expansion to 791 residents.

The July visit shifted to an unannounced infection‑control audit, the first required annual review since the pandemic.
