The purpose of this survey was to conduct a compliance inspection and investigate intake #GA50004467, with the investigation beginning on 2025-08-26 and completed on 2025-09-08.

Investigation was initiated based on intake #GA50004467, with onsite visit on 2025-08-26 and completed on 2025-09-08.
Findings
The facility failed to ensure staff providing hands-on personal services had current CPR certification, failed to administer medications according to physician orders for 2 residents, failed to obtain medications in a timely manner and notify physicians of unavailability, and failed to securely store controlled substances in a locked cabinet of substantial construction.
