The City of Yuba City’s “Courtyard” community care facility has faced a series of complaints between late 2020 and September 2023, each investigated by California Department of Social Services, Community Care Licensing Division. In the first investigation (February 2021), the evaluator noted that the administrator had taken reasonable steps to restore internet and cable service and found no evidence of the alleged disrepair, rendering the complaint unsubstantiated.
A second complaint, received in September 2022, raised concerns about staff training, supervision, resident safety, and accommodation. Evaluator Kerry Hiratsuka observed that the facility meets training requirements, keeps staff available, and follows established policies. Interviews with residents and staff yielded no corroborating evidence of the alleged violations, and the report concluded the allegations were unfounded. A related set of allegations—denials of restroom use, lack of privacy, and a resident’s fall—could not be verified because the evaluator could not interview the implicated former residents. Consequently, those claims were also deemed unsubstantiated.
In early 2023, two separate complaints were investigated. One alleged that staff caused a resident’s fall after placing an oxygen tank on the floor; the evaluator could not determine causation due to conflicting accounts, and the finding was unsubstantiated. Another complaint questioned the facility’s backup emergency services following a power outage. The evaluator confirmed that the facility had a written emergency plan, complied with regulatory requirements, and that residents with oxygen needs were adequately supported, leading to an unfounded conclusion.
The most recent complaint in August 2023 concerned staff allegedly forcing a resident to take medication. After interviewing the resident and staff, the evaluator found no evidence to support the allegation and marked it unsubstantiated. Across all five investigations, no deficiencies under Title 22 regulations were cited, and no corrective action was recommended. The pattern shows a facility consistently meeting regulatory standards, with complaints largely lacking substantiating evidence, and investigators confirming compliance with training, supervision, and emergency protocols.
